Located in northern MN, Cook Hospital specialized as a rural hospital with acute services including ER and ambulatory care. Attached is a small skilled nursing facility with residents cared by both medical staff and rehab. An outpatient rehabilitation center is attached to the hospital and provides community services including a rehabilitation gym. Therapy consisted of a combination of acute hospital visits, gym orientations, outpatient services, and skilled nursing screens/rehab. Close collaboration with MDs, and nursing allowed for efficient and safe patient care. In the rehab unit staffing included one front desk receptionist, 1 OT, 1 part-time SLP, 1 PTA, and 2 PTs. Daily scheduling as the recruiting therapist was 7am-3:30pm, Mon-Fri with 70-80% of care as outpatient, 10-15% hospital care, and 5-10% SNF focusing on interventions including preventative care, self-care management, manual therapy, orthopedic interventions, therapeutic exercise, modalities, and neuromuscular education as applicable.
